The
Ohio Department of Health (ODH) issued new immunization requirements in 2010.
One of those requirements is that students entering 7th grade must have had one
dose of Tdap (Tetanus, diphtheria, acellular pertussis) vaccine. Tdap is not
given before age 11 so most students have not had it yet. If they have had
Tdap, the school would not have the date on file.
ODH
has issued a second requirement in 2016 requiring incoming 7th grade students
to receive one dose of Meningococcal vaccine. A second vaccination will be
required prior to entry into Grade 12.
Please submit a record of the date (month-day-year) that a Tdap and the
Meningococcal vaccines were given before the school year begins in August,
preferably before school ends in May. Do not put off fulfilling the Tdap
and the Meningococcal requirements. State law requires that we exclude students
who do not meet the immunization requirements within 14 days of the beginning
of the school year.

Thank you for your help in meeting this requirement.
